The Denise Boulanger Scholarship

THE DENISE BOULANGER SCHOLARSHIP

ELIGIBILITY:

1. The applicant must be a member or child of a member in good standing of Local 5067.

2. The applicant should have applied to an institution of higher learning.

3. All information requested must be supplied by the applicant

HOW TO APPLY:

1. Carefully fill in all information requested. (Any false information will void this application)

2. Application must be complete. Incomplete applications will not be considered.

3. Applications must be returned to the Union Office by Wednesday October 17, 2007.

4. Applications for this scholarship will be reviewed by the Scholarship Committee.




RULES AND REGULATIONS:

1. Awards are for the current year and must be presented to the recipient (member or child) at the Union Office.

2. Please note: We will be publicizing the winners of the scholarships.

3. There will be two (2) scholarships available for $500 each.

4. The recipient will receive a check in the amount of the scholarship awarded once verification of registration for the semester at the respective school has been received by the Scholarship Committee. This scholarship money will remain designated for the winner for one (1) year, if not used in this time, the money shall be forfeited by the recipient and returned for future scholarships.

Instructions for filing:

1. All information requested must be supplied by the applicant. Any forms requiring signatures must be signed.
2. Applications must include:
   1. Required essay legibly written or typed.
   2. Completed application form.
3. Applicants must submit application form and related material to:

   Applications and material submitted are treated as confidential.  The deadline for applying has been extended to April 1, 2008.
   
   
4. Apply between January 1, 2008 - April 1, 2008.
5. Review of applications will be during the month of April.
6. Awards will be made in April for the next academic year.

Rules and Regulations & Qualifications

1. Applicants must be UNAP members or child of UNAP member (under age of 24).
2. Students must be enrolled in a degree granting institution and credit classes.
3. Students must be enrolled at least halftime.
4. Student must be UNAP member or child of a UNAP member at time of award.

Awards are for the current year.


Checks will be made payable to the individual. Please note: We will be publicizing the winners of the scholarships.

Process for Decision Making

1. A three-person committee will choose the winner(s) of the scholarship. The committee will be comprised of the three UNAP Vice-Presidents or their designees.
2. The essay will be judged based on form and content.
3. There will be up to three $1,000 scholarships awarded.
